What is a CEA Test?

The carcinoembryonic antigen (CEA) test measures the level of a specific protein, known as CEA, in your blood. This protein, which has sugars attached to it, can be produced by both normal and cancerous cells. Discovered in 1968, CEA was first identified in patients with colorectal cancer and in the normal fetal digestive system. While everyone is born with high levels of CEA, the levels naturally decrease with age. However, certain conditions, including cancer, can cause CEA levels to rise.

CEA is considered a tumor marker, a substance produced by some cancer cells that can be found in bodily fluids. While healthy cells also produce small amounts of these markers, high CEA levels may indicate the presence of certain types of cancer.

What Cancers Can a CEA Test Detect?

A CEA test does not diagnose cancer but is used in patients already diagnosed with certain cancers, primarily colorectal cancer. It is commonly used to monitor the progression of cancer, assess the effectiveness of treatment, and check for cancer recurrence.

 

In addition to colorectal cancer, CEA tests may be used for other types of cancer, such as breast, lung, bladder, ovarian, and thyroid cancer. This test is typically part of a broader diagnostic approach, often combined with physical examinations and imaging tests.

When is a CEA Test Needed?

While the CEA test isn’t used to diagnose cancer, it provides valuable information for patients who have already been diagnosed. Your healthcare provider may recommend a CEA test to:

  • Check if cancer has spread to other areas of your body.
  • Monitor how well cancer treatments like chemotherapy or radiation are working.

You might undergo a CEA test at different stages of your cancer treatment:

  • After diagnosis to establish a baseline.
  • During treatment to assess the effectiveness of therapy.
  • After completing treatment to monitor for any signs of cancer recurrence.

If you are in remission, your doctor may recommend regular CEA tests to detect any potential recurrence.

Test Details

Most CEA tests are blood tests. A healthcare professional will draw a small blood sample from one of your veins, usually in your arm. The sample is then sent to a lab for analysis to measure CEA levels.

In some cases, your provider may test other body fluids, such as:

  • Abdominal fluid (peritoneal fluid).
  • Chest fluid (pleural fluid).
  • Spinal fluid (cerebrospinal fluid or CSF).

 

In these cases, a needle is used to collect a small sample of the fluid from the chest or lower back.

Your healthcare provider will inform you of any specific preparations for the test. You may need to stop taking certain medications or supplements, such as aspirin, that could increase the risk of bleeding.

What to Expect During a CEA Test

During a standard blood test, your healthcare provider will:

  • Insert a small needle into a vein, usually in your arm.
  • Collect a small blood sample in a vial or test tube.

If other fluids need to be tested, your provider will:

  • Use local anesthesia to numb the area.
  • Insert a needle to collect fluid from your lower back or chest.
  • In some cases, they may use an X-ray to guide the needle for accurate placement.

The entire blood test process usually takes less than five minutes, while fluid tests may take around 30 minutes. Afterward, you can typically go home the same day.

Risks of a CEA Test

CEA tests are generally safe with minimal risk. For chest fluid tests, there is a small chance of:

  • Blood loss
  • Infection
  • Lung damage

However, complications are rare, and your healthcare provider will explain any potential risks before the test.

Side Effects of a CEA Test

For a blood test, you might experience mild discomfort or bruising at the site where the needle was inserted.

If fluid samples were taken from the chest or lower back, possible side effects include:

  • Dizziness
  • Headaches
  • Lightheadedness
