Nerve interventions involve procedures that target specific nerves to manage chronic pain conditions. These interventions include nerve root blocks, radiofrequency ablation (RFA), pulsed radiofrequency (PRF), coeliac plexus blocks/neurolysis, sympathetic blocks, and cryoablation of nerves. Each technique works by interrupting pain signals travelling through nerves to the brain, providing relief for patients with various pain conditions. The procedures typically involve placing needles near target nerves under image guidance, followed by administration of medication or application of energy to modify nerve function.

Indications for Nerve Interventions

These nerve intervention procedures may be considered for patients who experience persistent pain that has not responded adequately to conservative treatments or medication management.

Chronic Back and Neck Pain

Patients with persistent spinal pain originating from facet joints or nerve roots may benefit from nerve blocks or radiofrequency procedures. These conditions often result from degenerative disc disease, spinal stenosis, or arthritis.

Neuropathic Pain

Individuals suffering from nerve damage or dysfunction, such as peripheral neuropathy or post-herpetic neuralgia, may experience relief through targeted nerve interventions. These procedures can help modulate abnormal nerve signalling that causes persistent pain.

Cancer-Related Pain

Patients with pain from cancer or its treatments may receive relief through celiac plexus blocks or other nerve interventions. These procedures can address pain in the abdomen, pelvis, or other regions affected by tumour growth or treatment effects.

Complex Regional Pain Syndrome

This condition involves abnormal sympathetic nervous system activity, making sympathetic nerve blocks a potential treatment option. These blocks can interrupt the cycle of pain and reduce associated symptoms like swelling and skin changes.

Post-Surgical Pain

Some patients develop persistent pain after surgery that may respond to specific nerve interventions. These procedures can target nerves involved in post-surgical pain when other treatments have been ineffective.

Benefits of Nerve Interventions

Nerve interventions offer several advantages for patients with chronic pain conditions who have not found relief through other treatments.

Pain Relief

Patients often experience a reduction in pain intensity and frequency following nerve interventions. The degree of relief varies depending on the specific condition and procedure performed.

Reduced Medication Dependence

Many patients can decrease their reliance on pain medications after successful nerve intervention procedures. This reduction may help avoid medication side effects and concerns about long-term use of certain pain medications.

Diagnostic Value

Some nerve blocks serve both therapeutic and diagnostic purposes, helping identify pain sources. This diagnostic information can guide future treatment decisions and improve overall pain management strategies.

Minimally Invasive Approach

These procedures typically require only needles rather than surgical incisions, resulting in less tissue trauma. Most nerve interventions can be performed as outpatient procedures with minimal recovery time.

Targeted Treatment

Nerve interventions address specific pain-generating structures rather than affecting the entire body. This targeted approach may reduce systemic side effects compared to oral medications that circulate throughout the body.

Nerve Intervention Techniques

These injections target spinal nerve roots where they exit the spinal canal. The procedure involves delivering local anaesthetic and sometimes corticosteroid medication to the affected nerve root under imaging guidance. Nerve root blocks can provide both diagnostic information about the source of pain and therapeutic benefits by reducing inflammation around the nerve. They are commonly used for radicular pain patterns such as sciatica.

This technique uses heat generated by radiofrequency waves to create a controlled lesion on the nerve, disrupting pain signal transmission. A specialised needle is positioned adjacent to the target nerve under imaging guidance, then a radiofrequency current generates heat at the needle tip. RFA provides longer-lasting relief than nerve blocks and is frequently used for facet joint pain and other chronic pain conditions.

Unlike conventional RFA, PRF delivers short bursts of radiofrequency energy with cooling periods in between, modifying nerve function without creating a thermal lesion. The procedure uses similar equipment to RFA but applies energy differently to avoid permanent nerve damage. PRF may be preferred when treating mixed sensory and motor nerves or when preserving nerve function is particularly desirable.

These procedures target the coeliac plexus, a network of nerves in the upper abdomen that transmits pain signals from abdominal organs. Under imaging guidance, needles are positioned near the coeliac plexus for injection of local anaesthetic (block) or neurolytic agents like alcohol (neurolysis). These interventions are particularly effective for pain associated with pancreatic cancer and chronic pancreatitis.

These procedures interrupt the function of the sympathetic nervous system, which can become overactive in certain pain conditions. Depending on the target area, blocks may be performed on the stellate ganglion (neck), lumbar sympathetic chain (lower back), or ganglion impar (tailbone region). Sympathetic blocks often help diagnose and treat complex regional pain syndrome, vascular insufficiency, and certain neuropathic pain conditions.

This technique uses extreme cold rather than heat to temporarily disable nerve function and interrupt pain transmission. A specialised probe is positioned near the target nerve and rapidly cooled using gas expansion principles, creating an ice ball that affects the nerve. Cryoablation provides pain relief with potentially fewer complications than heat-based techniques and may be beneficial for treating peripheral nerve pain.

Preparing for the Procedure

  • Medical Evaluation: Patients undergo assessment of their medical history, current medications, and pain condition before any procedure. This evaluation includes a review of previous imaging studies, such as MRI or CT scans, to identify anatomical landmarks and confirm the suspected pain source. Blood tests may be ordered to check for bleeding disorders or infections that may affect procedure safety.
  • Medication Adjustments: Some medications may need to be temporarily discontinued before nerve interventions. Blood thinners such as warfarin, aspirin, and certain anti-inflammatory medications often require adjustment or temporary cessation. Patients taking diabetes medications may need modified dosing on the procedure day, especially if fasting is required.
  • Fasting Guidelines: Patients typically need to avoid food and drink for a specified period before the procedure. Solid foods are usually restricted for 6-8 hours pre-procedure, while clear liquids might be permitted up to 2-4 hours before. These restrictions help prevent complications related to sedation.

Step-by-Step Procedure

The skin over the procedure area is cleaned with an antiseptic solution. A local anaesthetic is then administered to numb the skin and deeper tissues at the needle entry site.

Fluoroscopy (real-time X-ray), ultrasound, or CT guidance is used to visualise anatomical structures and guide needle placement. Contrast material may be injected to confirm proper needle position near the target nerve. The choice of imaging technique depends on the specific procedure and anatomical considerations.

A specialised needle is advanced to the target nerve location. Once positioned correctly, the specific intervention is performed—medication injection for nerve blocks, application of radiofrequency energy for RFA/PRF, or extreme cold for cryoablation. For nerve blocks, the injected solution typically includes a local anaesthetic and may include other medications.

After completing the intervention, the needle is removed and a small dressing is applied. The patient’s response is assessed, checking for expected sensory changes in the targeted nerve distribution. Patients remain under observation for a short period before discharge with post-procedure instructions.

Post-Procedure Care and Recovery

Immediate Post-Procedure Care

Patients remain in a recovery area for observation, typically for 30-60 minutes after the procedure. During this time, vital signs are monitored and any adverse reactions are assessed. Temporary numbness, weakness, or other sensory changes in the affected area are common effects of the local anaesthetic and can help confirm that the correct nerve was targeted.

Activity Restrictions

Physical activities are limited for a short period following nerve interventions. Most patients can resume light activities within 24-48 hours but should avoid strenuous exercise, heavy lifting, and prolonged sitting for several days. Spinal procedures often require more activity limitations than peripheral nerve interventions.

Pain Management

Temporary increases in pain may occur as the local anaesthetic wears off before the therapeutic effect begins. Over-the-counter pain relievers may be recommended for post-procedure discomfort. Application of ice to the injection site may help reduce local soreness.

Follow-up Evaluation

A follow-up appointment is scheduled to assess the outcome and discuss next steps in pain management, typically 2-4 weeks after the procedure. This evaluation determines whether additional interventions are needed or if the current treatment plan should continue.

Potential Risks and Complications

Nerve intervention procedures carry certain risks despite their minimally invasive nature. Patients may experience temporary soreness at the needle insertion site or brief increases in pain following the procedure. Infection, bleeding, and bruising are uncommon but possible complications. Nerve-related complications such as temporary numbness, weakness, or increased pain in the distribution of the treated nerve can occur. Allergic reactions to medications or contrast agents used during the procedure may develop in some patients.
